NEW PATIENT SCHEDULING, REGISTRATION & INSTRUCTIONS

A step-by-step guide to completing your patient registration*

1. Call now to schedule an appointment with one of our providers, at 505-322-2510.

2. You will receive an email that will prompt you to register for our patient portal. This email will provide you with your username to access the portal.

3. The temporary password you will need to enter (unless otherwise stated) will be: welcome1

4. You will be asked to confirm your personal information and then create your own password. 

5. Once logged on, the information that will need to be filled out prior to your first appointment can be found by clicking on the “Questionnaires” tab on the left side of your home screen. 

6. You will be guided through a series of questions and surveys. Please allow 30-45 minutes to complete this process.  

   
   

In order to sign onto / access the portal AFTER initial registration, please visit www.power2patient.net.

* Tip: Using a desktop computer or laptop will be the best way for you to complete the questionnaires and surveys.
